Lawmakers want to hear from you – restaurant operators, employees, and customers – about how the last year has impacted the restaurants you love and the action you need them to take now.

I hereby consent that the National Restaurant Association, an Illinois not-for-profit corporation qualified as an IRS 501(c)(6) association, its affiliates and their representatives, successors and assigns (collectively, the “Association”), shall have the right to publish, modify, edit, transmit, license and/or otherwise use my name, statements, image, likeness and/or voice in any audio and/or video recordings, photographs, digital images, and the like (collectively, my “Voice and Likeness”), they have taken or made of me in connection with the Association’s Restaurants Act program (“Program”) or in which I may have been included, for publicity, promotion, internet usage or any other lawful purpose, or in reproductions thereof.
I hereby waive all claims for any compensation for use of my Voice and Likeness and waive any right that I may have to inspect and/or approve the finished product or use or promotion thereof. I further waive any right to inspect or approve the finished Program. I understand that my participation in the Program is voluntary and will not be compensated in any way. I hold harmless, and release and forever discharge, the Association and its licensees, from any claim, demand, or cause of action which I, or any other person acting on my behalf may have by reason of the use of my Voice and Likeness.
